MEADOW GOLD DAIRIES' HAWAII "SPRING TO WELLNESS" SWEEPSTAKES
OFFICIAL RULES

No Purchase Necessary.
Meadow Gold Dairies' Hawaii's "Spring to Wellness" Sweepstakes ("Game") is open only to legal residents of the state of Hawaii, 18 years or older. Offered only in the state of Hawaii; void where prohibited by law. Employees of Meadow Gold Dairies' Hawaii ("Sponsor"), Dean Foods, Kinara Advertising ("Independent Judges" or "Judges"), any other entities involved in the implementation/administration of the Game, their respective parent companies, subsidiaries, affiliates and the immediate families (parents, children, siblings, spouses) of such persons regardless of where they live and members of their same households (whether related or not) are not eligible to participate or win.

Game begins on or about April 15th, 2007 and is scheduled to end on June 11th, 2007 ("Game Period"). Regardless of scheduled end date, Game will end when all game codes have been distributed (as discussed below), subject to the code check deadline of July 9th, 2007 (for mail-in code checks, July 13th, 2007) and the prize claim deadline of July 20th, 2007.
How To Play:
1. Get A Game Code. Find a game code on promotional labels of specially marked Meadow Gold Dairies' Hawaii Viva 2%, 1% and Fat Free milk gallon containers available at participating retailers in the state of Hawaii. Each promotional label will contain one game code. In addition, game cards (each with a game code) will be distributed at community events as well as via product demonstrations at grocery stores in select areas of the state of Hawaii during Game Period. Limit one game card/game code per person per event/product demonstration. List of events will be available at www.lanimoo.com (list is subject to change without notice).
  
  OR, FOR A GAME CODE WITHOUT PURCHASE (and a copy of these Official Rules), send a self-addressed, stamped envelope to "Spring to Wellness" Game Code Request, P.O. Box 1880, Honolulu, HI 96805, to be postmarked by June 11th, 2007 and received by June 14th, 2007. Limit one game code per valid request. Each request must be mailed separately.
  
  Game pieces (promotional labels or game cards)/game codes available only while supplies last.
2. Check Your Game Code. Peel or scratch off indicated area of promotional label or game card (as appropriate) to reveal a WORD (the word is the game code). Then, between April 15th, 2007 at 12:01 a.m. Hawaiian-Aleutian Standard Time (HST) and July 9th, 2007 at 12:01 p.m. HST, visit the Meadow Gold Dairies' Hawaii website on the Internet at www.lanimoo.com to see if your game code is a winner (subject to verification). Note: Sponsor's computer is the official clock for purposes of the Game. First, register at the website by entering your full name, complete residential mailing address, daytime phone number, e-mail address and date of birth. The collection and use of your registration information will be governed by the privacy policy of www.lanimoo.com (i.e., the Meadow Gold Dairies' privacy policy), the terms of which are incorporated herein by reference. If you do not agree to the privacy policy, do not register for the Game at the website or otherwise provide any personal information on the website; instead, check your game code by mail (see below). Limit one registration per person. Second, after registering, follow the prompts on-screen to enter your game code in the indicated field. Be sure to enter game code exactly as printed on promotional label or game card: game code is case sensitive and spaces (if any) must be included. Incomplete, garbled or otherwise inaccurate game codes will NOT be accepted for verification; and, if entered improperly, an error message will appear on screen. If your game code is a winner, a winning message will appear on screen and will be accompanied by a brief description of the prize won. Winning message only indicates that you are a potential winner, subject to verification; screen shots will NOT be accepted as evidence of winning; you are only a winner after receiving written notification to such effect, addressed to you personally, from the judges. If a non-winning game code is entered, a losing message will appear on-screen instead. All game cards and promotional labels will also contain a coupon redeemable for 75 cents off a purchase of a gallon of Meadow Gold or Viva milk. If you do not have access to the Internet, to have your game code checked, you must submit a game code check request via mail. In your request, legibly print (or type) the above indicated registration information (full name, complete residential mailing address, daytime phone number and date of birth) for purposes of determining your eligibility to participate in Game, on a 3"x5" paper and mail with the original promotional label/game card (NOT a copy) to "Spring to Wellness" Game Code Check, P.O. Box 1880, Honolulu, HI 96805, to be received by July 13th, 2007. Be sure to detach the Meadow Gold Dairies' Hawaii Milk Coupon portion of promotional label/game card BEFORE submitting game code check request so that you may redeem cents-off coupon. Independent Judges will check the game code, and will notify you ONLY if your game code is a winner. IMPORTANT: You may check your game code on the website OR by mail, BUT NOT VIA BOTH METHODS.
Prize Claim (for persons who check game code on website): Legibly print (or type) your full name, complete residential mailing address, daytime phone number and date of birth on a 3"x5" paper and mail with original promotional label or game card (NOT a copy) to: "Spring to Wellness" Prize Claim, P.O. Box 1880, Honolulu, HI 96805, to be received by July 20th, 2007. Be sure to detach Meadow Gold/Viva Milk Coupon portion of promotional label/game card BEFORE submitting prize claim. You are advised to make and retain a copy of the promotional label/game card for your records. After verification of promotional label/game card (and game code), winner will be notified by mail. For receipt of notification from judges: allow approximately 2 – 3 weeks after submission of prize claim. UNCLAIMED AND FORFEITED PRIZES WILL NOT BE AWARDED.
Prizes/Odds of Winning ("Odds")***:
1. Grand Prize (4) – Each Grand Prize Package is a health and beauty makeover, including the following:
  - Three-Month membership to 24-HR Fitness (if there is no 24-HR Fitness location within island of winner's residence, Sponsor will instead award a three-month membership to a comparable health club [as determined by Sponsor in its sole discretion] within island of winner's residence)
  - Fashion Consultation with ADASA staff (1 hour) and $250 Gift Certificate from ADASA.
  - One year subscription to Smart Magazine.
  - Meadow Gold Dairy Products, with a total value of $329.
  - Nutrition Consulting with registered dietician on staff (1.5 hours) from Meadow Gold Dairies.
  - Hair cut and makeup application from Paul Brown Spa and Salon (if there is no Paul Brown Spa and Salon location within island of winner's residence, Sponsor will instead award this prize at a comparable salon [as determined by Sponsor in its sole discretion] within island of winner's residence)
  Total ARV of each Grand Prize package is $1,400. Odds are 1:60,500.
  
  Winner will be solely responsible for all other costs/expenses associated with accepting the Grand Prize. Prize components may be subject to availability, usage deadlines and other restrictions. Without limitin the foregoing, ADASA gift certificate is subject to terms and conditions of use, as indicated thereon.
2. Second Prize (500) – 14-Day membership to 24hr Fitness (if there is no 24-HR Fitness location within island of winner's residence, Sponsor will award a 14-Day membership to a comparable health club [as determined by Sponsor in its sole discretion] within island of winner's residence). ARV: $100 each. Odds are 1:60.
3. Third Prize (10) – Tri-Pack of Paul Brown Products including shampoo, conditioner and styling product. ARV: $50 each. Odds are 1:3,000.
4. Fourth Prize (50) – One-year subscription to Smart Magazine. ARV: $21.00 each. Odds are 1:600.
5. Fifth Prize (24,200) – Coupon for a Free Single Serve Pint of Meadow Gold Dairies' Milk. ARV: $1.29 each. Odds are 1:10.
***Odds of winning Grand and Fifth Prizes are based on random seeding of game codes within the 242,000 total game pieces (promotional labels and game cards). Odds of winning Second, Third or Fourth Prizes are based on random seeding of game codes within 30,000 game cards. No Second, Third or Fourth Prizes will be seeded among the game codes on promotional labels.

Limit of one Grand, Second, Third or Fourth Prize per person.
Limit of five (5) Fifth Prizes per person.
Conditions on Participation. Winners of Grand Prizes must complete, sign and return an affidavit of eligibility/liability release/publicity release (where legal) within (10) days of date of prize notification. In the event of noncompliance with this requirement, Grand Prize will be forfeited. If prize notification for any prize is returned as undeliverable or such prize is unclaimed for any reason, such prize will be forfeited. Decisions of Sponsor and judges are final and binding in all respects. By participating, you agree to abide by and be bound by all such decisions and these Official Rules. No prize transfer or substitution, except by Sponsor in its sole discretion due to unavailability of advertised prize (or prize component) for any reason and only then for prize/prize component of equal value. Winner is solely responsible for any and all taxes on prize(s). Each promotional label/game card may only be used once and is redeemable for a maximum of one prize (if properly verified). Game codes may only be obtained/used/redeemed in accordance with these Official Rules by the person who obtained the promotional label/game card bearing such game code; and, game code (and the promotional label/game card bearing such game code) is not to be reproduced, transferred, assigned, purchased or sold (by auction or otherwise). Any such use of promotional labels/game cards/game codes is strictly prohibited. Game codes have no cash value nor do they have any value other than as specifically set forth in these Official Rules. Submission of prize claims/game code check requests is the sole responsibility of participant. All submissions become the sole property of Sponsor and will not be returned. Sponsor and its agencies are not responsible for (a) lost, late, stolen, incomplete, damaged, illegible, misdirected prize claims/registrations/game code requests/game code check requests or postage due mail; (b) any incorrect or incomplete Game information available on the website (including whether one's game code is a potential winner) or otherwise, whether caused by equipment or human error or unauthorized human intervention; or (c) any computer, telephone, cable, satellite, network, programming, electronic, or Internet or hardware or software malfunctions, failures, connections, availability or garbled or jumbled transmissions, service provider/Internet/web site/use net accessibility or availability, traffic congestion, or unauthorized human intervention. If, for any reason, the Meadow Gold Dairies' Hawaii website is corrupted such that persons cannot check their game codes or otherwise participate in the Game online, Sponsor shall have the right at its sole discretion to cancel, terminate, modify or suspend the online component of the Game. In such event, so as to determine whether game codes are potential winners, Sponsor may require ALL participants to submit their original promotional labels/game cards via the mail-in game code check request procedure [see (2b) above] and/or Sponsor may take any other actions Sponsor deems necessary in its sole discretion so as to conduct the Game in a fair and appropriate manner which is consistent with the spirit of these Official Rules. Notice of such action by Sponsor will be posted on www.lanimoo.com.
Verification. All prize claims and/or game code checks are subject to verification by judges. Promotional labels/game cards/game codes are void and will be rejected if not obtained through authorized, legitimate channels or if any part is determined by the judges in their sole discretion to be illegible, mutilated, counterfeited or tampered with in any way or if materials contain printing, typographical, mechanical or other errors. Sponsor's liability for any irregular promotional label/game card/game code is limited to replacement with another randomly selected promotional label/game card/game code provided supplies last. If because of error or for any other reason, there are more claims than prizes offered in any prize category, a random drawing will be held from among all eligible claimants in the appropriate prize category to award only the prizes as set forth in these Official Rules for such prize category (which were not previously awarded). In all cases, only the number of prizes stated in these Official Rules are available to be won in the Game. Any attempt to forge promotional labels, game cards or game codes or to tamper with the Meadow Gold Dairies' Hawaii website in any way will result in disqualification of person attempting same and forfeiture of any prizes won by such person; and, such person will also be subject to prosecution to the full extent of the law. CAUTION: ANY ATTEMPT BY A PARTICIPANT TO DELIBERATELY DAMAGE THE WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE GAME IS A VIOLATION OF CRIMINAL AND CIVIL LAW. SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM SUCH INDIVIDUAL TO THE FULLEST EXTENT OF THE LAW (AND TO DISQUALIFY SUCH INDIVIDUAL FROM THE GAME WITH FORFEITURE OF ANY PRIZES WON BY SUCH INDIVIDUAL).
Releases. By participating, participant releases and agrees to hold harmless Meadow Gold Dairies' Hawaii, Dean Foods, Kinara Advertising, their respective parent companies, subsidiaries, affiliates, any othe entities involved in the implementation or administration of the Game, including prize suppliers, as well as the officers, employees, directors, representatives or agents of any of the above entities from any and all liability arising from participation,in the Game or receipt, acceptance, possession, use/misuse of any,prizes. Acceptance of prize(s) constitutes one's permission for Sponsor to,use his/her name, photo, voice, likeness and/or other indicia of identity for,advertising/ publicity and trade purposes, world-wide and on the world,wide web, without further compensation and without review or notice.
Winners' List. For a list of winners of major prizes, send a self-addressed stamped envelope to "Spring to Wellness" Winners List, P.O. Box 1880, Honolulu, HI 96805 upon request. Requests must be received by August 31st, 2007.
